Information for Tenants Needing Assistance

Tenants will be referred through partner agencies and be required to complete an application that will:

(i) be reviewed by the subsidy committee,

(ii) enable the committee to determine individuals in greatest need of assistance, and

(iii) calculate the amount and duration of subsidy necessary for the home requested

The listed Landlord will need to provide:

  (i) a copy of the lease showing the tenants name, the property address, the amount of rent, and the term of the lease

  (ii) proof that the tenant is in arrears

    (iii) a completed W9 form
